Introduction Nasopharyngeal carcinoma (KNS) is a type of cancer that is known to have a poor prognosis due to its proximity to the base of the skull and other vital structures. KNS shows higher invasive and metastatic characteristics compared to other head and neck cancer. This cancer is often a challenge in diagnosis and treatment, especially because of the ability of cancer cells to spread to the surrounding tissue and to other parts of the body. Understanding the molecular mechanisms underlying the development and progression of KNS is crucial for the development of effective therapeutic strategies . The role of metalloproteinase-9 (MMP-9) in the degradation of the extracellular matrix (ECM) has been implicated in the invasion and metastasis of various cancers, including KNS.
Research Purposes This study aims to determine the expression of MMP-9 in the case of nasopharyngeal carcinoma. The process of invasion, metastasis, and angiogenesis of cancer cells is a series of complex steps that involve the collaboration of various proteolytic enzymes secreted by tumor cells or host cells, which function to the extracellular matrix component. Extracellular matrix degradation will encourage tumor cell invasion into the surrounding tissue, including blood vessels or lymphatic. MMP-9 plays an important role in this degradation process, becoming a key step in tumor invasion, metastasis, and induction of tumor tissue vascularization . The expression of MMP-9 has been shown to be associated with poor prognosis and aggressive behavior in various cancers.
Method This study was conducted with a descriptive design using the immunohistochemical coloring method to detect MMP-9 expression in 30 nasopharyngeal carcinoma tissue that never undergo radiotherapy, chemotherapy, or a combination of both. This is done to ensure that the results obtained are purely from the characteristics of tumors without the influence of previous therapy . The immunohistochemical analysis was performed using a monoclonal antibody against MMP-9, and the results were evaluated using a semi-quantitative scoring system.
Results From the results of the study, patients with nasopharyngeal carcinoma are mostly found in the age group 41-50 years and 51-60 years, each as much as 30%. Most patients are male (73.3%) with non-keratinizing squamous cell carcinoma (53.3%). Immunohistochemical analysis shows that the expression of MMP-9 protein detected in KNS tissue does not show a significant difference between histological types, primary tumors (T), metastasis of lymph nodes (N), and KNS stadium (P> 0.05) . The results suggest that MMP-9 expression is not significantly associated with the clinical characteristics of KNS.
Conclusion MMP-9 plays an important role in the development of nasopharyngeal carcinoma, although no significant differences are found in MMP-9 expression based on tumor characteristics. Further research is still needed to fully understand the mechanism that underlies the invasion and metastasis of this cancer .
