Chronic diseases, such as hypertension, diabetes, and coronary heart disease, are one of the biggest challenges in the health system in Indonesia. These diseases not only affect the quality of life of patients but also impose a significant financial burden on patients, hospitals, and social security organizing agencies (BPJS Health). The financial load caused by treatment of these diseases, especially drug costs, can be a significant problem for patients, hospitals, and social security organizing agencies. Therefore, analysis of drug claims for chronic diseases is very important to understand the pattern of drug use and optimize cost management.

Research Purpose and Significance

This study aims to analyze chronic drug claims for BPJS patients at Prof. Dr. Chairuddin Panusunan Lubis Medan (RSP CPL Medan). Through this study, it is expected to provide insight into drug expenditure related to chronic diseases and help the hospital and BPJS in the planning and management of health costs. The study will also help to identify the factors that affect the cost of chronic disease drugs and provide recommendations for optimizing the use of drugs and managing costs.

Methodology and Data Collection

This study uses a quantitative descriptive approach with retrospective observational study design. BPJS patient drug claim data at RSP CPL Medan is collected from medical records and drug use data in pharmaceutical installations during the April to June 2023 period. For sampling, a probability method with proportional stratified random sampling is used. The data collected is then grouped based on inclusion criteria, then analyzed using Statistical Product and Service Solution (SPSS) software version 22 and Microsoft Excel. Data analysis was carried out using the Kruskal-Wallis test and Mann-Whitney test.

Research Result and Findings

From the results of the analysis, found a significant difference between the number of prescribed drugs and differences in the cost of chronic disease drugs for BPJS patients. It is known that the more drugs are prescribed, the higher the difference in cost. In addition, there is a significant difference between the type of polyclinic and the difference in the cost of chronic diseases for BPJS patients, which shows a variety of drug costs in various polyclinics. However, no significant differences were found between the number of secondary diagnoses and differences in the cost of chronic diseases for BPJS patients.

Analysis and Implications of the Study

This finding highlights the importance of understanding the factors that affect the cost of chronic disease drugs. Variability on the cost of drugs between polyclinics shows that policies in each polyclinic may differ in the management of recipes and expenses. Therefore, hospitals need to evaluate treatment policies and cost management in order to provide more efficient and economical services for patients.

In terms of BPJS Health, the results of this study can be used as a basis for formulating strategies in managing claims and cost control. Monitoring of drug use patterns and related costs can help prevent waste and ensure that patients get optimal treatment.

Conclusion and Recommendations

This study shows that several factors affect differences in the cost of chronic disease drugs for BPJS patients at RSP CPL Medan, including the number of prescribed drugs and polyclinic types. Further efforts are needed in optimizing the use of drugs and managing costs so that the health system in Indonesia can run more effectively and sustainably. Through a better understanding of this drug claim pattern, it is expected to improve the quality of health services for the whole community.

This study has several limitations. Firstly, the study only focuses on BPJS patients at RSP CPL Medan, which may not be representative of all patients with chronic diseases in Indonesia. Secondly, the study only analyzes the cost of chronic disease drugs and does not consider other factors that may affect the cost of treatment, such as the cost of hospitalization and other medical expenses.
