Introduction

Metastatic spine disease (MSD) is a serious condition with a poor prognosis and low life expectancy. Determining the life expectancy of MSD patients becomes crucial for appropriate treatment and support strategies. This study aims to examine the relationship between serum levels of alkaline phosphatase (ALP) as a prognostic marker for MSD patients.

This study is a retrospective analytic descriptive study conducted at Adam Malik Haji Hospital Medan from September 2014 to December 2018. ALP level data from 28 MSD patients were analyzed to see their relationship with the patient's survival rate for 3-6 months, 6-12 months, and more than 12 months.

Results

The results showed that there was a significant difference in the survival rate of MSD patients based on ALP levels (p = 0.001). Patients with ALP levels of 20-140 IU/L have a life expectancy of more than 12 months, while patients with ALP levels 141-238 IU/L have a life expectancy of 6-12 months. Patients with ALP levels of more than 238 IU/L have a life expectancy of 3-6 months.

Further analysis shows that the average ALP levels are lower in groups of patients with a life expectancy of more than 12 months compared to groups of 6-12 months (78.77 (± 28.48) vs 182 (± 30.22)) IU /L. ALP content is also lower in groups of 6-12 months compared to groups 3-6 months (182.67 (± 30.22) vs 321.50 (± 32.92)) IU/L. This finding shows a statistically significant relationship (p = 0,000).

Discussion

The results of this study support the use of ALP serum levels as a prognostic marker in MSD patients. High ALP levels show poor prognosis and lower life expectancy. This finding can help doctors in determining the appropriate treatment and support strategies for MSD patients.
