Introduction

Nasopharyngeal carcinoma (KNF) is a type of cancer that affects the nasopharynx, the upper part of the throat behind the nose. It is a significant health concern in Indonesia, with over 13,000 new cases reported each year. The disease is characterized by its aggressive nature and high mortality rate. Understanding the molecular mechanisms underlying KNF is crucial for developing effective treatment strategies. One of the key proteins involved in the development of KNF is P38 MAPK, a protein kinase that plays a crucial role in various cellular processes, including proliferation, differentiation, migration, and apoptosis.

Background

P38 MAPK is a member of the mitogen-activated protein kinase (MAPK) family, which is involved in the regulation of cellular responses to various stimuli, including stress, cytokines, and growth factors. Previous studies have shown that P38 MAPK plays a dual role in cancer development, with both tumor-suppressive and tumor-promoting effects. In the context of KNF, P38 MAPK has been implicated in the regulation of tumor growth, invasion, and metastasis.

Methodology

This study aimed to investigate the relationship between P38 MAPK expression and histopathological type in patients with KNF. A total of 126 patients diagnosed with KNF at the Adam Malik Hajj General Hospital between February and December 2019 were included in the study. The diagnosis was based on a combination of clinical history, physical examination, and histopathological biopsy. Immunohistochemical techniques were used to evaluate the expression of P38 MAPK in the biopsy tissue samples. The immunoreactive score was used to determine the level of P38 MAPK expression, with scores ≥ 4 considered positive or overexpressed.

Results

The results of the study showed that KNF most commonly occurred in the age group 41-60 years (57.1%), male (71.4%), with non-Keratinizing SCC histopathology (79.4%), and stage IV (54.8%). Further analysis revealed that the expression of P38 MAPK was more commonly found in patients with Non-Keratinizing SCC histopathology (81.0%), T3-T4 tumor stage (97.1%), N3 lymph nodes (100%), and Clinical Stadium III-IV (96.8%).

Discussion

The findings of this study suggest that there is a significant correlation between the expression of P38 MAPK and the size of the primary tumor, enlargement of the neck lymph nodes, and clinical stage. However, no significant relationship was found between the expression of P38 MAPK and the KNF histopathology type. This suggests that P38 MAPK may play a role in the development of KNF, particularly at an advanced stage. The high expression of P38 MAPK in non-keratinizing SCC may indicate higher tumor aggressiveness.

Conclusion

In conclusion, this study provides new insights into the relationship between P38 MAPK expression and histopathological type in patients with KNF. The findings suggest that P38 MAPK may play a role in the development of KNF, particularly at an advanced stage.
