The rats were given a high-fat diet for 12 weeks and a subcutaneous 10 mg/kgBW testosterone injection for 4 weeks. Puguntano extract was given orally for 10 days. Then, PI3K, Interleukin-6, VEGF, and PSA parameters were evaluated using the ELISA method. In addition, the prostate index value was calculated and the prostate histopathological examination was carried out. Data analysis was carried out using one-way ANOVA test for normal distribution and Kruskal-Wallis test if the distribution was not normal.

The results showed that there was a significant increase in interleukin-6 levels (p = 0.024), PSA levels (p <0.001), and prostate index values (p <0.001) after treatment. In addition, there was a significant improvement in prostate histopathology examination of stroma and epithelial cells (p <0.001). Nevertheless, the administration of puguntano extracts did not have a significant effect on PI3K levels (p = 0.069) and VEGF (P = 0.274).

Has a positive effect in reducing inflammatory markers, increasing PSA levels, prostate index, and improving prostate histopathology in obesity rats that experience BPH. This finding shows the potential of puguntano leaf extract as an alternative therapy in the management of BPH, especially in individuals who experience obesity.
