This study aims to determine the relationship between the severity of hemophilia with the formation of inhibitors in hemophilia patients at the Adam Malik Hajj Hospital Medan. Methodology

This study is an observational study with a cross-sectional design carried out from November to December 2014. A total of 22 hemophilia patients were included in this study. The study used the Bethesda Inhibitor Assay (BIA) to assess the existence of inhibitors.

Results

Of the 22 research subjects, the average age of the patient is 28.68 years. A total of 13 subjects (59.1%) have a family history of hemophilia, and 5 subjects (22.7%) have a family history of inhibitors. The examination results of FVIII levels show an average of 2.25%, with the lowest level of 0.8% and the highest 7.8%. Based on the degree of severity, 17 subjects (77.3%) are in a medium category with an average inhibitor level of 3.04 BU. A total of 20 subjects underwent substitute therapy using concentrate Factor VIII and Cryoprecipitate. There is a significant relationship between the degree of severity and the formation of inhibitors (p = 0.004; p <0.05). However, this study found no significant differences in inhibitor titers based on the severity of hemophilia (p = 0.284) and there was no significant difference at the average inhibitor levels based on the severity of hemophilia (p = 0.068). The correlation between the degree of hemophilia severity and inhibitor levels is also not significant (r = 0.097).

Conclusion

From the results of the study, it can be concluded that there is a significant relationship between the severity of hemophilia with the formation of inhibitors. However, no significant correlation is found between the degree of severity and inhibitor levels in hemophilia patients. This shows that although the severity of hemophilia can affect the formation of inhibitors, other factors may contribute to determining the levels of inhibitors in the patient's body. Therefore, appropriate supervision and management for hemophilia patients remains important to prevent further complications.
