Introduction

The COVID-19 pandemic has had a significant impact on public health worldwide, particularly for individuals with comorbid conditions such as type 2 diabetes mellitus (T2DM). This study aims to describe the lipid profile in confirmed COVID-19 patients who also suffer from T2DM at H. Adam Malik Hospital Medan during the March 2020 to December 2021 period.

Diabetes can cause secondary hyperlipidemia, and in COVID-19 cases, dyslipidemia is considered a result of biological and pathological processes triggered by SARS-COV-2 infections. Since the beginning of the pandemic, there have been reports of changes in lipid profiles that show a positive relationship between the severity of SARS-COV-2 infection and these changes. This encourages further research to analyze the condition of lipids in the body of COVID-19 patients who also have T2DM.

The lipid profile is a crucial aspect of a patient's health, particularly for those with T2DM. Method

This study uses a descriptive design with a cross-sectional approach. Secondary data are collected from inpatients who are confirmed COVID-19 and suffer from T2DM at H. Adam Malik Hospital during the specified period. The data obtained were then analyzed using the SPSS version 25 application to get a comprehensive picture of the patient's lipid profile.

Results

Of the 372 subjects studied, the majority of patients were male (57.3%). A total of 193 people had a body mass index (BMI) between 18.5 - 24.9 kg/m² (51.9%), with an average age of 57.5 ± 10.4 years and a length of median treatment for 8 days. The most commonly found comorbidity was hypertension, with 189 patients (50.8%). A total of 176 people (47.3%) experienced severe COVID-19 conditions, while 225 patients (60.5%) managed to get out of the hospital.

Laboratory Data

Based on laboratory data, 327 patients (87.9%) experienced dyslipidemia. A total of 31 patients (8.3%) showed disorders of mild liver function. The average HBA1C level of the patient was recorded at 9.0 ± 2.5%, with an average fasting blood glucose level of 221.7 ± 122.2 mg/l and blood glucose levels of 2 hours post-drandial at 261.7 ± 125.8 mg/L. The average total cholesterol content was 151.2 ± 52.2 mg/l, triglyceride levels 147.5 ± 99.1 mg/l, HDL 33.1 ± 14.6 mg/L, and LDL 97.4 ± 44.5 mg/L. Data showed the Alt median of 25 μ/L and the average AST 30.45 ± 27.42 μ/L.

Conclusion

The results showed that total cholesterol, triglycerides, and LDL were lower in T2DM patients who experienced critical conditions due to COVID-19 and in patients who died. These findings indicate a complex relationship between diabetes, dyslipidemia, and the severity of COVID-19 infections. Further research is needed to understand the mechanism that underlies changes in this lipid profile and to formulate a better treatment strategy for patients with T2DM infected with COVID-19.

With a better understanding of the picture of this lipid profile, it is expected to improve patient health management in the future and help in preventing further complications.
