This study aims to investigate the implementation of discharge planning for inpatients at the Hospital of the University of North Sumatra, with a focus on the challenges and recommendations for improvement.

Discharge planning is a multidisciplinary process that involves healthcare professionals, including nurses, doctors, and social workers, working together to ensure a seamless transition of care from the hospital to the community. The results of this study showed that the implementation of discharge planning was categorized as poor, with 47 nurses (67.1%) revealing that they sometimes did not have time to provide health education to patients due to high workload. This finding indicates a significant challenge in the application of effective discharge planning in hospitals. The lack of time to provide education can lead to patients not fully understanding the process of further treatment after returning home, increasing the risk of health complications and readmissions.
